Dear Needlework Buyer:

Pre-registration forms for name badges must be received no later than April 12, 2009. Also, please be advised that all shops must provide three (3) items proving business existence. These "proof of business" items must accompany your pre-registration form. Acceptable proofs are any three of the following: 

(1) Recent paid invoices to suppliers
(2) Tax ID# (state or federal) - copy of form
(3) Cancelled check (copy) with business name
(4) Copy of employee withholding tax forms (Letterheads or business cards will not be accepted.) 

If you supplied this information during the past year, you are not required to submit again at this time.
